I hereby clarify that I am currently living in the Netherlands, and
that I have filled out this form as Patta stated. I also hereby state
that I have only signed up once for the adidas raffle at Patta and that
my registration can be seen as invalid if I did sign up for multiple
shoes.

You can only sign up ONCE for ONE ITEM. If you sign up multiple times
your entry will NOT be accepted. Once your email is received, we will
respond on Friday, March 15th if you are among the winners. Winners can
purchase their pair in person only on Saturday, March 16th or
Sunday, March 17th at Zeedijk 61. You will be required to bring
valid identification under the same name as submitted in the entry as proof.
We will NOT accept third-party pickups.
